Clinical evidence update — CAPTAIN risk ratio 2.1; p=0.004 vs robotic RP
Profound Medical’s clinical data deepen TULSA’s real-world and randomized evidence, while lower-cost MRI compatibility remains only a future opportunity.
Profound is a commercial-stage medical-device company trying to turn TULSA-PRO’s clinical differentiation into broader hospital adoption; its installed base reached 84 systems by the end of Q2 2026, while recent operating momentum has centered on converting new accounts and installed systems into recurring procedure revenue.
The filing adds breadth and credibility to the evidence base. Mayo Clinic data extend TULSA beyond controlled-trial settings, while the CARE Registry shows the platform can support focal, subtotal and whole-gland treatment. The most commercially useful piece is the randomized CAPTAIN comparison: whole-gland TULSA produced fewer complication-related hospitalizations and more than twice the likelihood of preserving both erectile function and pad-free continence versus robotic surgery. 〔0〕
The new real-world data are supportive but not cleanly one-directional. The Florida experience reported 100% pad-free continence and 87.5% preservation of erections sufficient for penetration beyond 12 months, but the Rochester study reported biopsy-confirmed recurrence in 22% of patients. 〔1〕 〔2〕 That leaves the oncologic durability question open, especially because the company itself notes single-center design, patient selection, treatment heterogeneity and the need for continued follow-up.
The lower-field MRI result is an adoption option, not an adoption milestone. Technical work on Siemens’ 0.55T Free.Max platform showed 40% better temperature-measurement precision and targeting accuracy of 1.0 ± 0.9 mm, which could eventually reduce infrastructure barriers for hospitals. But the filing explicitly says this does not establish FDA clearance, clinical validation or commercial availability, so it does not yet expand the sellable installed base.
Relative to expectations, this is incremental evidence rather than a surprise commercial step-change. CAPTAIN’s favorable functional result was already public, and the symposium setting was expected to produce supporting clinical presentations. The fresh details strengthen the case for TULSA’s quality-of-life advantages and flexibility, but they do not provide new revenue, reimbursement, regulatory clearance or confirmed installations.
Bottom line: The filing improves the clinical foundation behind Profound’s adoption story, particularly against robotic surgery, but it remains evidence-building rather than commercially transformative. The next meaningful test is whether this stronger evidence converts into utilization, installations and revenue.
