GEP-NET enrichment — 75% PFS at 18 months; 39% ORR in 57 SSTR2-positive GEP-NET patients
Perspective Therapeutics’ trial data strengthens the case for an SSTR2-selected GEP-NET registration study, but remains an early, retrospective subgroup readout.
Perspective is a clinical-stage radiopharmaceutical company trying to move its targeted alpha therapy bamzireotide from dose-finding into a registration-enabling program. The October 23, 2026 ESMO update had already established the broad story: durable disease control, response rates of 33% and 50% in Cohorts 2 and 3, and a planned path toward Phase 3 site preparation.
The new information is not that bamzireotide works, but where it appears to work best. In the retrospective subgroup of 57 GEP-NET patients whose target lesions all expressed SSTR2, 18-month progression-free survival was 75%, while disease control reached 92% in Cohort 2 and 100% in Cohort 3. 〔0〕
| Metric (source) | NANETS 2026 | Comparison |
|---|---|---|
| ORR, SSTR2-positive GEP-NETs (Efficacy table) | 39% | 13% approved radiotherapy |
| PFS at 18 months, SSTR2-positive GEP-NETs (Efficacy table) | 75% | ~65% approved radiotherapy |
| Disease control, Cohort 2 (Efficacy table) | 92% | 89% all treated patients |
| Disease control, Cohort 3 (Efficacy table) | 100% | 95% all treated patients |
That sharpens the development strategy, rather than proving a new efficacy step-change. The company is now considering an enrichment strategy that would focus a future registration study on GEP-NET patients with SSTR2 expression in every target lesion. That is a meaningful improvement in trial design and regulatory positioning, but it is based on a small, retrospective subgroup, and the headline response figures include some unconfirmed responses. 〔1〕
Safety remains supportive and does not add a new obstacle. Across 68 treated patients, there were no dose-limiting toxicities, treatment-related discontinuations, or serious adverse events related to study medication during the treatment-emergent window; however, long-term renal follow-up is still ongoing. 〔2〕
The regulatory timeline is more concrete, but still conditional. Perspective is targeting an end-of-Phase 2 FDA meeting in the first quarter of 2027 and expects a Phase 3 start in 2027 if FDA feedback and the remaining clinical, regulatory, and operational work align. The company's cash position had previously been described as sufficient for planned milestones into late 2027, so this update advances the program without immediately creating a disclosed financing requirement. 〔3〕
Bottom line: This is a modestly positive refinement of an already favorable clinical story: the strongest signal is becoming narrower and more actionable, but it remains an early subgroup analysis awaiting regulatory validation and prospective confirmation.
