Above the bar — 50% ORR in Cohort 3; 72% PFS at 18 months
Perspective Therapeutics’ trial update strengthens VMT-α-NET’s path to Phase 3, combining durable disease control with encouraging safety.
Perspective is a clinical-stage radiopharmaceutical company trying to turn its lead SSTR2-targeted alpha therapy, VMT-α-NET, from an encouraging early-stage program into a registrational Phase 3 asset. Earlier 2026 updates had already shown response deepening and acceptable safety, so the direction was not a surprise; this filing supplies the fuller Cohort 2 dataset, the first meaningful Cohort 3 signal, and longer durability.
The efficacy signal holds up as the study expands. Across 46 patients in Cohort 2, ORR was 33%, while the higher-dose Cohort 3 produced a 50% ORR in 20 patients. The company also reports disease control of 89% and 95%, respectively, with 72% progression-free survival at 18 months across all cohorts. 〔0〕 〔1〕 This is above the usual early-development bar because the efficacy is not just a handful of isolated responses: disease control remains broad and appears durable. The caveat is that Cohort 3 is only 20 patients and includes unconfirmed responses, so the higher response rate is promising rather than established.
Safety continues to support the program’s therapeutic-window argument. No dose-limiting toxicities, severe Grade 4 or 5 treatment-emergent events, or adverse-event-driven discontinuations were reported. 〔2〕 That matters for a targeted radiopharmaceutical, where the ability to deliver repeat doses without forcing patients off treatment is central to the commercial and clinical case. Still, 29% of patients experienced Grade 3 adverse events, including Grade 3 renal events in two patients, so longer renal follow-up remains an important qualification rather than a footnote.
The filing advances VMT-α-NET from “promising Phase 1/2a data” toward execution of a Phase 3 plan. Perspective says it expects to complete Phase 3 site-startup preparations around year-end 2026 and initiate Phase 3 in 2027, subject to FDA alignment. 〔3〕 This is not regulatory clearance or a registrational result, but the filing makes the next development step more concrete and gives the clinical data a clear purpose.
Bottom line: The update is better than a routine confirmation: efficacy remains credible across a larger dataset, the higher dose looks active, and tolerability still supports advancement. It materially strengthens the Phase 3 story, though small cohorts, interim follow-up, unconfirmed responses, and pending FDA alignment keep it from being decisive proof of efficacy or approval potential.
