Below the bar — DMC said the trial was unlikely to meet its overall-survival primary endpoint
Geron discontinued its IMpactMF clinical trial after monitors judged the overall-survival endpoint unlikely, narrowing imetelstat’s growth story.
Geron is shifting from development risk to commercial execution. RYTELO is already approved for certain lower-risk MDS patients in the U.S. and European Union, while IMpactMF was the major effort to extend imetelstat into relapsed/refractory myelofibrosis; the company had been positioning that trial as a potential additional growth driver.
The key efficacy test has effectively failed. Geron is discontinuing the Phase 3 IMpactMF trial after its independent Data Monitoring Committee concluded that it was unlikely to meet the primary endpoint of overall survival. 〔0〕 This is a direct setback to the myelofibrosis expansion case, not merely a delay or a routine interim update.
The setback is meaningful because the trial was a substantial registrational effort. IMpactMF enrolled 327 patients with intermediate-2 or high-risk myelofibrosis who had relapsed or were refractory to a JAK inhibitor, testing imetelstat against best available therapy for a survival improvement. 〔1〕 The filing gives no efficacy numbers, hazard ratio, or subgroup explanation, so the precise reason for the failure remains unclear.
This was not framed as a new safety problem. The company says there were no new safety signals requiring a change to imetelstat’s known safety profile or additional monitoring. 〔2〕 That removes one possible explanation, but it does not soften the central conclusion that the study was unlikely to demonstrate its survival benefit.
The commercial RYTELO story now carries more weight. Management redirected attention to growing U.S. demand and expanding access for eligible lower-risk MDS patients outside the U.S. 〔3〕 Because the timing of an IMpactMF update was anticipated for the second half of 2026, the catalyst itself was partly known; the negative clinical outcome is the new information.
Bottom line: This removes a major potential indication from Geron’s near-term growth narrative and leaves execution of the existing RYTELO business as the clearer path forward. The later data release may explain the result, but it cannot restore the trial’s primary objective unless it reveals an unexpected alternative finding while the trial is being discontinued.
