Miss — Revenue $13.83M vs ~$13.93M consensus; EPS -$0.52 vs ~$-0.49
CVRx earnings missed on revenue and EPS as U.S. payer pressure and weaker sales productivity outweighed broader coverage progress.
CVRx is a commercial-stage medical-device company trying to expand adoption of its single-product Barostim therapy through broader Medicare Advantage coverage, clinical evidence and a larger U.S. sales footprint. Humana and Devoted Health had already added Medicare Advantage coverage policies in 2026, while the BENEFIT-HF trial was enrolling, so the strategic direction was known before this report.
The quarter missed a modest pre-release benchmark. Revenue was $13.832 million, below the published consensus of roughly $13.93 million, while diluted EPS was -$0.52 versus about -$0.49 expected.
| Q3 2026 | Q3 2025 | Consensus | |
|---|---|---|---|
| Revenue (Financial Statements) | $13.832M (-5.8%) | $14.690M | ~$13.93M |
| Net loss per share (Financial Statements) | -$0.52 | -$0.49 | ~-$0.49 |
| Gross margin (Financial Results) | 87% | 87% | |
| Cash and equivalents (Balance Sheet) | $58.746M |
The core commercial problem remains unresolved. U.S. revenue fell 4% year over year despite active implanting centers rising to 262 from 250 and sales territories expanding to 56 from 50. Management attributed the decline to lower sales-force productivity and a prolonged challenge with one of its largest payers.
International weakness adds to the execution strain. European revenue dropped 27% to $0.9 million, with units falling to 38 from 50. The combination of U.S. reimbursement friction, softer productivity and European contraction means the larger commercial footprint is not yet translating into demand growth.
Cost controls helped contain the damage, but did not change the loss-making model. R&D fell 16%, and management said it had reduced non-commercial costs while continuing to invest in sales and reimbursement teams. Still, SG&A rose 2%, operating losses widened to $13.0 million, and net cash used in operating and investing activities increased to $11.0 million. 〔0〕
The balance sheet is becoming less forgiving. Cash declined to $58.7 million from $75.7 million at year-end, while long-term debt rose to $58.7 million from $49.5 million and shares outstanding increased to 28.5 million from 26.3 million. The company retains funding capacity, but continued losses are being financed with a thinner cash cushion, more debt and a larger share count.
The evidence and coverage pipeline remains the offset, not the current result. New Medicare Advantage policies, BENEFIT-HF enrollment and favorable real-world Barostim data could improve the long-term adoption story, but this quarter shows that coverage expansion has not yet overcome payer and sales-execution bottlenecks. The CFO transition is also largely known, with John Landry's appointment previously announced and effective November 6, 2026. 〔1〕
Bottom line: This was a modest earnings miss that exposes continued friction converting reimbursement progress and a larger sales footprint into revenue. The long-term commercial thesis remains intact, but the near-term operating story weakened rather than advanced.
