Beat — Adjusted EPS $2.44 vs ~$2.14 consensus; revenue $560.4M vs ~$552.1M
ICU Medical’s Q3 earnings beat consensus as Infusion Systems growth and margins improved, while full-year guidance stayed unchanged near the top end.
ICU Medical is still working through the Smiths Medical integration and regulatory overhang while shifting the business toward its higher-growth Infusion Systems and Consumables franchises; Vital Care remains the drag after the IV Solutions divestiture. The prior quarter had already lifted full-year expectations, and the company’s 2025 warning letter remains unresolved, although the older 2021 warning letter was closed earlier in 2026. The quarter beat the published bar, but not by enough to rewrite the story. Adjusted diluted EPS was $2.44 versus a published consensus of about $2.14, while revenue was $560.4 million versus roughly $552.1 million expected. The company itself says Infusion Systems was ahead of expectations while the rest of the business was generally in line.
| Q3 2026 | Q3 2025 | Consensus / guide | |
|---|---|---|---|
| Revenue (Financial Highlights) | $560.4M (+4.4%) | $537.0M | $552.1M |
| Adjusted diluted EPS (Non-GAAP reconciliation) | $2.44 (+20.2%) | $2.03 | $2.14 |
| Adjusted EBITDA (Adjusted EBITDA reconciliation) | $113.9M (+7.5%) | $105.9M | |
| GAAP gross margin (Financial Highlights) | 42% (+5 pts) | 37% | |
| FY26 adjusted EPS guidance (Outlook) | $8.60–$9.00 |
The quality of the beat improved materially. Revenue growth was concentrated in the businesses ICU Medical is trying to build: Consumables rose 5% organically and Infusion Systems rose 11% organically, while the margin expanded to 42% from 37%. 〔0〕 That combination points to better operating execution rather than a revenue-only quarter, although the company’s adjusted figures still exclude substantial restructuring, remediation, amortization and other items.
The portfolio remains uneven. Vital Care organic revenue declined 10% in the quarter, so the headline revenue growth is not broad-based. The contrast is important: the core growth engines are performing well, but the legacy and post-divestiture pieces are still shrinking, leaving the company dependent on Infusion Systems and Consumables to carry the recovery.
Guidance was reaffirmed, not raised. ICU Medical expects to finish toward the high end of its existing FY26 ranges, but it left adjusted EPS at $8.60–$9.00 and adjusted EBITDA at $415–$435 million. 〔1〕 That is better than a deterioration in outlook, but it limits the incremental surprise because the prior quarter had already established the higher guidance range.
Cash generation and leverage support the transition. Nine-month free cash flow reached $172.7 million versus $55.9 million a year earlier, while principal debt repayments reduced long-term debt from $1.27 billion at year-end to $1.13 billion at September 30. The balance sheet is moving in the right direction, though the company still reports 2.04x net leverage and continues to carry integration and regulatory execution risk.
Bottom line: This was a clean operating beat led by Infusion Systems and stronger margins, but unchanged guidance and continued Vital Care weakness keep it a modest improvement to the existing recovery story—not a wholesale reset.ewerkers
