TransMedics Group, Inc. (TMDX) · Nov 5, 2026 · Earnings
Beat — Adj. EPS $0.51 vs ~$0.30 consensus; revenue $203.0M vs ~$178.1M
TransMedics earnings delivered a major revenue beat and lifted 2026 guidance, but rising investment is still pressuring margins and EPS.
TransMedics is scaling its organ-preservation platform into a broader transplant infrastructure business: OCS usage is expanding through the U.S. National OCS Program, while PAD Aviation adds owned logistics capacity and ENHANCE, DENOVO, and OCS Kidney extend the future product pipeline. Its prior 2026 outlook already anticipated roughly 22%–25% growth excluding PAD Aviation, so the key question was whether demand could accelerate without the new clinical programs contributing yet.
The quarter materially cleared the operating bar. Revenue reached $203.0 million, up 41%, with organic growth of 35%; the published pre-release consensus was roughly $178.1 million, making this a sizable top-line beat. Adjusted diluted EPS was $0.51 versus consensus near $0.30, although the comparison is less clean because earnings were held back by higher investment and interest expense.
| Figure | Q3 2026 | Q3 2025 | Consensus |
|---|---|---|---|
| Revenue (Financial Highlights) | $203.0M (+41%) | $143.8M | ~$178.1M |
| Organic revenue (Revenue reconciliation) | $193.6M (+34.6%) | — | — |
| Adjusted diluted EPS (Financial Highlights) | $0.51 | $0.67 | ~$0.30 |
| Operating margin (Financial Highlights) | 14.2% (-220 bps) | 16.4% | — |
| Service revenue (Financial Highlights) | $89.6M (+60%) | $56.1M | — |
The growth is broad enough to strengthen the core story, not just the acquisition story. PAD Aviation contributed $9.4 million in the quarter, but revenue excluding that contribution still grew organically 34.6%, while organic service revenue rose 42.8%. That indicates the underlying NOP and transplant-logistics engine is doing most of the work rather than the headline being purely acquisition-driven. 〔0〕
The trade-off is that expansion is consuming more of each revenue dollar. Gross margin was broadly stable at 58.5%, but operating expenses jumped to $90.5 million from $61.3 million as TransMedics funded OCS Kidney, next-generation OCS, clinical programs, headcount, infrastructure, and PAD Aviation. As a result, nine-month adjusted operating margin fell to 12.8% from 20.2%, and adjusted EPS declined year over year despite the quarterly revenue beat. This is not a demand problem in the filing; it is the cost of building the larger platform.
The guidance raise confirms the acceleration, but the magnitude is partly acquisition-assisted. Full-year revenue guidance increased to $764–$774 million, including approximately $14 million from PAD Aviation, versus prior guidance of $737–$757 million excluding PAD Aviation. The underlying business guidance excluding PAD Aviation rose to $750–$760 million, so management raised the core outlook by $13 million at the midpoint and added a separate European aviation contribution. 〔1〕
The pipeline now has more concrete entry points, but none is commercial revenue yet. FDA IDE approvals for ENHANCE Heart Part B and DENOVO Lung, plus continued access for 90 additional ENHANCE Part A patients, improve the path toward new heart and lung indications. The first European logistics tender in Italy also validates the international expansion plan, but these developments remain execution catalysts rather than current-period earnings drivers. 〔2〕
Bottom line: This was a genuine earnings beat with a meaningful guidance raise and strong organic demand, strengthening TransMedics’ expansion story. The main limitation is that margins and per-share earnings are moving lower while the company spends heavily to build the next phase of growth, so the beat advances the story without resolving its execution trade-off.
