Beat — Adjusted diluted EPS $0.44 vs published consensus ~$0.34
Medline earnings beat on strong sales and tariff refunds, while raising organic-growth guidance and reaffirming EBITDA amid margin pressure.
Medline is a large medical-surgical distributor expanding its delivery and manufacturing network while rebuilding Northern California capacity after the June Tracy distribution-center fire; it has leased more than 1.6 million square feet of replacement space, including a facility scheduled for January 2027. The prior setup already called for roughly 9%-10% organic growth and $3.3-$3.4 billion of 2026 adjusted EBITDA.
The quarter beat on the cleanest earnings benchmark. Adjusted diluted EPS was $0.44 versus a published consensus of roughly $0.34, while adjusted EBITDA rose 11.8% to $960 million. The beat is real, but it was helped by $81 million of net IEEPA tariff-refund benefits, so it says less about recurring profitability than the headline suggests.
| Q3 2026 | Q3 2025 | Consensus | |
|---|---|---|---|
| Net sales (Financial Highlights) | $7,856 (+10.4%) | $7,115 | |
| Adjusted EBITDA (Reconciliation) | $960 (+11.8%) | $859 | |
| Adjusted diluted EPS (Adjusted EPS reconciliation) | $0.44 | ~$0.34 | |
| Gross margin (Income Statement) | 24.5% | 26.4% | |
| Operating margin (Income Statement) | 6.4% | 8.1% | |
| Free cash flow (Free Cash Flow reconciliation) | $1,804 (+49.6%) | $1,206 |
The underlying growth engine remains healthy, but profitability is not keeping pace. Organic sales grew 10.5%, and Medline Brand adjusted EBITDA increased 14.9%; however, Supply Chain Solutions sales rose 13.9% while its adjusted EBITDA fell 10.2%, with margin dropping to 4.4% from 5.5%. Companywide gross margin and operating margin also contracted sharply.
Management raised the sales outlook, not the profit outlook. Full-year organic-sales growth is now expected at 10.0%-10.5%, up from 9.0%-10.0%, while adjusted EBITDA remains $3.3-$3.4 billion. That is a positive demand and execution signal, but the unchanged EBITDA range implies the extra revenue is not yet translating into better operating leverage.
Cash generation gives Medline room to keep investing and return capital. Nine-month operating cash flow reached $2.2 billion and free cash flow $1.8 billion, supporting a new $1 billion repurchase authorization and a $750 million prepayment on the 2033 refinancing term loan. Those actions strengthen the balance-sheet and capital-allocation story, although they do not fix the weaker margins in Supply Chain Solutions.
Bottom line: This is better than expected because demand is strong, the sales outlook moved higher, and cash generation is robust. But the business still needs to convert that growth into durable margin improvement once tariff refunds and other unusual benefits fade, so the filing advances the story without rewriting it.
