Genprex biomarker update sharpens REQORSA trials but stays unproven

Biomarker validation update — Low TROP2 linked to resistance in 50% of cell lines and organoid models

Genprex’s biomarker update links low TROP2 to REQORSA resistance, potentially improving patient selection while leaving efficacy and validation unresolved.

Genprex is a cash-constrained clinical-stage biotech trying to advance REQORSA through Phase 2 lung-cancer trials while using biomarkers to improve patient selection. The direction was already public: earlier 2026 disclosures described higher TROP2 as potentially associated with better REQORSA outcomes, and the company announced a Roche Diagnostics collaboration in August to validate TROP2 testing.

The new information is more specific resistance evidence, not a new clinical result. Across the preclinical work, approximately half of the NSCLC cell lines and half of the patient-derived organoid models showed primary resistance, with low TROP2 consistently associated with that resistance. The filing says, “Protein-expression analyses consistently identified low TROP2 expression as a marker associated with resistance across multiple preclinical models.” 〔0〕

That could make the trials more interpretable, but it does not yet prove REQORSA works better in selected patients. The supporting biopsy analysis from treated patients is directionally helpful, and manipulating TROP2 changed treatment sensitivity in experiments. But the filing itself says prospective clinical evaluation is still required, so this remains a patient-selection hypothesis rather than an efficacy milestone. 〔1〕

The practical trade-off is narrower enrollment in exchange for a potentially cleaner signal. Excluding low-TROP2 patients could reduce apparent resistance and improve the odds of seeing activity in future NSCLC studies, but it may also shrink the addressable trial population and does nothing to remove Genprex’s broader execution, funding, or enrollment challenges. The company’s latest reported position included ongoing Phase 2 work, expected biomarker-validation completion around the end of 2026, and a stated need for additional capital.

Bottom line: This advances the REQORSA development story modestly by giving Genprex a clearer way to select patients and interpret trial results. It is incremental confirmation of an existing biomarker thesis, not yet evidence that the therapy’s clinical odds have materially changed.
