Cencora earnings beat, but FY27 EPS outlook is only in line

Beat — Adjusted EPS $4.66 vs ~$4.56 consensus

Cencora’s earnings beat Q4 expectations, helped by OneOncology and stronger margins, while FY27 guidance points to steady—not accelerating—growth.

Cencora is shifting from a traditional pharmaceutical distributor toward a broader specialty-pharma and provider-services platform, using OneOncology and a new segment structure to drive higher-margin growth. The quarter shows that strategy is working operationally, but the initial fiscal 2027 outlook is more steady than surprising. Published estimates clustered around roughly $4.56-$4.58 of adjusted Q4 EPS and $87.95-$88.16 billion of revenue, so the reported $4.66 and $88.4 billion were a narrow beat.

The core quarter was better than expected, not merely flattered by accounting noise. Revenue rose 5.5%, while adjusted operating income increased 22.1% and adjusted EPS rose 21.4% to $4.66. Gross-margin expansion and OneOncology’s contribution did the heavy lifting, although adjusted operating expenses also grew 23.5% as the acquisition was absorbed.

OneOncology is producing the intended mix shift. U.S. Healthcare Solutions operating income grew 22.0%, and the company explicitly attributes the margin improvement to the February 2026 acquisition of OneOncology. That is important because Cencora is trying to make specialty and provider services a faster-growing, higher-margin engine rather than simply adding more low-margin distribution volume.

The new fiscal 2027 outlook validates the direction but does not reset expectations higher. Management guided to 3%-5% revenue growth, 4%-6% adjusted operating-income growth and $19.40-$19.75 of adjusted EPS. The midpoint of the EPS range is about $19.58, modestly below the roughly $19.76 published estimate, while specialty-services operating income is expected to grow 18%-21% versus only 2%-4% for distribution. That supports the strategic repositioning, but the consolidated outlook is broadly an in-line execution case rather than a clear acceleration.

The cost of the transition is becoming more visible. Net interest expense rose 75.1% because debt and variable-rate loans helped finance OneOncology, and adjusted operating expenses rose faster than revenue in the quarter. The company also expects roughly $3.5 billion of adjusted free cash flow against $1.0 billion of capital spending, leaving room for debt service, buybacks and the newly raised dividend, but the acquisition-led model is not free.

The 10% dividend increase is supportive but secondary. The higher quarterly dividend signals confidence in cash generation, yet the business read is still dominated by whether OneOncology and the reorganized specialty segment can keep delivering faster profit growth without letting acquisition costs and interest expense absorb the gains.

Bottom line: Cencora delivered a real but narrow Q4 beat and showed that OneOncology is improving the earnings mix. The FY27 outlook keeps the strategic story intact, but its midpoint is roughly in line with—or slightly below—what analysts already expected, so this advances the story modestly rather than rewriting it.
