Beat — Adjusted EPS $2.09 vs ~$1.65 consensus
CVS Health earnings beat Q3 expectations as Aetna margins improved and the company raised 2026 adjusted EPS and cash-flow guidance.
CVS is in the middle of a Health Care Benefits margin recovery, shifting toward stronger government insurance economics while managing elevated medical-cost trends and pressure in its pharmacy businesses. That recovery delivered more than the market expected: published Q3 estimates were about $1.65 of EPS and $104.72 billion of revenue, versus CVS’s $2.09 adjusted EPS and $108.4 billion of revenue.
| Metric | Q3 2026 | Q3 2025 | Consensus / prior guide |
|---|---|---|---|
| Revenue (Consolidated results) | $108.4B (+5.4%) | $102.9B | ~$104.72B |
| Adjusted EPS (Consolidated results) | $2.09 | $1.60 | ~$1.65 |
| Health Care Benefits adjusted operating income (Segment results) | $912M (+190.4%) | $314M | |
| Medical benefit ratio (Segment results) | 91.3% (-150 bps) | 92.8% | |
| Operating cash flow, nine months (Cash Flow statement) | $15.3B | $7.2B | Prior floor: $11.5B |
| FY2026 adjusted EPS guidance (Guidance) | At least $8.20 | $7.90–$8.10 |
The core insurance recovery is materially better, not just cosmetically improved. Health Care Benefits adjusted operating income nearly tripled to $912 million, while the medical benefit ratio fell to 91.3% from 92.8%, meaning less of each premium dollar was consumed by medical costs. The improvement was driven primarily by the government business, even as total medical membership declined to 26.1 million from 26.7 million. CVS said, “Adjusted operating income increased significantly in the three months ended September 30, 2026 compared to the prior year primarily driven by improved underlying performance in the Government business.” 〔0〕
The guidance increase confirms management has more confidence in the recovery. CVS lifted adjusted EPS guidance to at least $8.20 from a prior $7.90–$8.10 range and raised operating-cash-flow guidance to at least $15.5 billion from at least $11.5 billion. The cash-flow revision is especially meaningful because nine-month operating cash flow already reached $15.3 billion. “The Company is increasing its full-year 2026 GAAP diluted EPS, Adjusted EPS and cash flow from operations guidance” 〔1〕
The rest of the portfolio is holding together, but it is not uniformly accelerating. Pharmacy & Consumer Wellness lifted adjusted operating income 10.6% despite a 2.0% revenue decline, helped by prescription volume and Rite Aid asset contributions. Health Services revenue grew 9.5%, but adjusted operating income rose only 2.7%, constrained by client price reductions. Those results support the raise, but they leave the insurance margin recovery as the main engine of the quarter rather than evidence of broad-based high-growth execution.
The headline GAAP improvement overstates the underlying year-over-year change. GAAP EPS rose to $1.68 from a $3.13 loss largely because the prior-year period included a $5.7 billion goodwill impairment. The cleaner signal is adjusted EPS rising from $1.60 to $2.09 and adjusted operating income increasing 21.2%; that still represents a genuine operational beat, just not the dramatic GAAP swing implied by the headline.
Bottom line: This is a clear step forward for CVS’s margin-recovery story: the insurance business improved sharply, the quarter beat expectations, and cash generation prompted another guidance increase. The result strengthens the existing turnaround narrative without rewriting the company’s broader story, because pharmacy economics and elevated medical-cost risks remain in the background.
