Below the bar — iADRS and CDR-SB failed; no meaningful treatment-placebo difference
Acumen’s Alzheimer’s trial missed its primary and key secondary endpoints, prompting the company to stop sabirnetug and conserve cash.
Acumen entered this readout as a clinical-stage Alzheimer’s company whose value centered on sabirnetug, while its newer Enhanced Brain Delivery program remained preclinical and targeted an IND filing in mid-2027.
The lead drug failed its core test, not just a secondary measure. ALTITUDE-AD missed the primary iADRS endpoint at both 35 mg/kg and 50 mg/kg, and CDR-SB and other key secondary endpoints also failed to reach statistical significance. The company says neither dose showed a meaningful difference from placebo.
The biomarker result does not rescue the clinical readout. Sabirnetug reduced amyloid plaque load significantly at both doses, but that biological effect did not translate into slower cognitive or functional decline over 18 months. That is the central failure for a disease-modifying Alzheimer’s program: target engagement was present, while the patient benefit the trial was designed to demonstrate was absent. 〔0〕
Acumen is effectively removing its lead clinical asset from the pipeline. The company plans to cease sabirnetug development, including the open-label extension, and shift attention toward strategic options and the EBD program. That is a major change from the pre-filing story, where sabirnetug was the active Phase 2 value driver and EBD was the follow-on option. 〔1〕
The failure lands harder because the balance sheet was already constrained. Acumen’s September 30, 2026 filing showed $78.6 million of cash and marketable securities and said those resources were expected to fund operations only into early 2027, with substantial doubt about continuing as a going concern. The announced cash-conservation measures therefore look less like routine discipline and more like an immediate need to preserve optionality for EBD, partnerships, or other strategic outcomes. 〔2〕
The remaining EBD option is earlier, riskier, and not a replacement clinical asset yet. Acumen can point to the program’s preclinical brain-delivery work and planned mid-2027 IND target, but this filing provides no human efficacy data or financing plan for advancing it. The additional fluid biomarker and imaging data may clarify mechanism, but management does not expect them to change the failed efficacy conclusion. 〔3〕
Bottom line: This is a decisive setback: Acumen’s lead Alzheimer’s drug showed plaque reduction but failed to slow disease, forcing a development shutdown and a cash-preservation-led strategic reset. The EBD program keeps a potential future path alive, but the company’s near-term clinical story has been materially impaired.
