Above the bar — HO lesion volume fell 8.09 cm³ vs rose 9.14 cm³ on placebo; nominal p=0.003
Mirum’s clinical trial data shows Atebrioz reduced abnormal bone growth in younger FOP patients, supporting a 2027 pediatric label expansion.
Mirum is extending a newly approved FOP franchise into younger children: Atebrioz was approved in September 2026 for patients aged 12 and older, while the company continues testing it in younger pediatric cohorts.
The efficacy signal clears the bar for a label-expansion study. In 25 children aged 6 to <12, zilurgisertib produced an 8.09 cm³ mean reduction in heterotopic-ossification lesion volume versus a 9.14 cm³ increase with placebo, a 14.0 cm³ treatment difference favoring the drug (Efficacy Results). 〔0〕 That is a clean placebo-controlled result in the exact age group Mirum wants to add, rather than merely a safety or pharmacokinetic readout.
The functional result makes the finding more meaningful than an imaging-only win. Clinician-assessed joint mobility improved by 0.80 points on CAJIS with zilurgisertib versus 0.09 points with placebo; the filing also frames that against reported worsening of roughly 0.5 points per year in untreated FOP patients (Efficacy Results). This supports the business case that the treatment may preserve function, not just reduce abnormal bone volume.
Safety does not appear to block the expansion path, but the dataset remains small. All 25 patients completed the double-blind period and entered the open-label extension, and no treated patient experienced growth plate closure (Study Description; Safety). 〔1〕 〔2〕 The limitation is that the evidence comes from only 12 treated children over 24 weeks, with full results still to come at a medical meeting.
This is partly confirmation, not a wholly new thesis. Cohort 1 already supported FDA approval in older patients, so the direction of evidence was established; today’s release adds the important detail that the effect extends to ages 6 to <12 and gives Mirum a stated first-half 2027 sNDA path. The younger-child result is therefore a meaningful incremental de-risking of the franchise rather than a first proof of concept.
Bottom line: The filing materially strengthens Atebrioz’s pediatric expansion story: efficacy is clear, function points the same way, and no new safety issue emerged. It advances the regulatory case, though the ultimate label decision still depends on the planned sNDA and review.
