Below the bar — Primary endpoint missed; MACE 8 vs 1, P=0.043
Longeveron’s clinical trial update adds a statistically significant MACE signal, but the failed primary endpoint still clouds the FDA path.
Longeveron is a cash-constrained, clinical-stage biotech trying to advance one cell therapy across HLHS, Alzheimer’s disease, pediatric dilated cardiomyopathy, and aging-related frailty; after the September 2026 topline miss, it had already begun emphasizing cost containment and strategic options while awaiting deeper ELPIS II analysis.
The primary efficacy failure remains intact. ELPIS II still did not show a statistically meaningful improvement in right-ventricular ejection fraction at 12 months, the endpoint that had been expected to anchor the HLHS regulatory case. 〔0〕 The detailed result was also based on Month 12 CMR data from only 28 of the 40 randomized infants, with a baseline-adjusted difference of -1.36 percentage points and p=0.68.
| ELPIS II measure | Standard of care | Laromestrocel | Comparison |
|---|---|---|---|
| Infants with at least one major cardiac event | 9 | 3 | — |
| Total major cardiac events | 22 | 3 | Rate ratio 0.16; p<0.001 |
| Total major adverse cardiac events | 8 | 1 | p=0.043 |
| RVEF change from baseline | -2.6 pts | -3.3 pts | Adjusted difference -1.36 pts; p=0.68 |
The secondary safety and event data are materially better than the September readout suggested. The newly disclosed analysis shows only 1 MACE in the laromestrocel arm versus 8 in standard of care, reaching statistical significance; the earlier topline update had described a lower event count but not a statistically significant result. 〔1〕 That gives Longeveron a more credible clinical-outcomes signal to take to regulators, while the small 40-infant trial and failure on the primary endpoint leave the evidence insufficiently clean for an obvious approval path.
The FDA question is now the central business issue. Longeveron says it will request an FDA meeting to determine next steps, rather than presenting a defined confirmatory-trial or approval route. That matters because the company’s cash position was previously expected to fund operations only into the fourth quarter of 2026, making any additional HLHS development plan dependent on financing, partnering, or further cost reductions.
Bottom line: This update improves the HLHS story versus the initial topline release, but it does not repair the failed primary endpoint. The program remains alive, yet its regulatory and funding path is still unresolved.
