60 Degrees Pharmaceuticals babesiosis update adds cures but no pivotal readout

Babesiosis program update — Hospital study remains blinded; 3 expanded-access cures confirmed

60 Degrees Pharmaceuticals’ babesiosis update strengthens relapsing-disease evidence and maps a faster FDA path, but the pivotal hospital study remains blinded.

60 Degrees Pharmaceuticals is a commercial-stage drug developer using marketed tafenoquine as the backbone for a potential babesiosis treatment, with relapsing and severe acute disease as the near-term regulatory engine and chronic disease as a diagnostic-heavy, exploratory program. The filing adds meaningful evidence around the edges of that strategy, but does not yet answer the central efficacy question.

Relapsing disease is the clearest advance. The company now reports three confirmed expanded-access cures, a 45-case reconstruction linking longer uninterrupted treatment with lower retreatment risk, and six treatment-free responses among seven published tafenoquine-atovaquone cases. The signal is encouraging, but it remains built largely from uncontrolled cases, retrospective modeling and selected refractory patients rather than a randomized efficacy result. 〔0〕 (Program update; Relapsing disease)

The hospital study is operationally de-risked, not clinically de-risked. Enrollment is capped at 30, follow-up is scheduled to finish in early January 2027, and the DSMB found no safety issue requiring a stop. But the efficacy allocation is still blinded, so the filing supplies no evidence that tafenoquine actually accelerates recovery versus placebo. 〔1〕 (Randomized Hospital Study) 〔2〕 (What the Interim Review Changed — and What It Did Not)

The chronic program is currently a bottleneck, not a validation point. Early screening found genus-level signals in 2 of 9 patients, but zero were positive on the species-specific B. microti assays, and no patient yet qualifies for the primary per-protocol analysis. That leaves open whether the signals reflect low-level infection, atypical Babesia, transient detection or unrelated prior exposure. 〔3〕 (B-FREE Screening Update)

The regulatory story is more flexible but still conditional. The company now frames either a regular supplemental application after a positive randomized result or a potential accelerated-approval route based on relapsing-disease evidence and post-treatment molecular negativity. The submitted national-priority review request could shorten review timing, but it cannot substitute for proof of effectiveness or manufacturing readiness. The next major decision point remains the hospital unblinding and the planned FDA meeting in late January 2027. 〔4〕 (Regulatory)

Bottom line: This is a broader and more coherent babesiosis package, especially in relapsing disease, but the filing does not deliver the pivotal result needed to change the program from promising evidence to demonstrated efficacy. The chronic study adds uncertainty, while January 2027 remains the real inflection point.
