ARC-20 data — 12-month PFS 69% in first-line combo; 14.5-month median PFS with cabozantinib
Arcus Biosciences’ new clinical data support casdatifan across kidney-cancer treatment lines, though small cohorts still leave Phase 3 proof ahead.
Arcus is concentrating its oncology strategy on casdatifan, aiming to make it a backbone treatment across multiple lines of clear cell kidney cancer; the company is already advancing PEAK-1 and has said a first-line PEAK-20 Phase 3 study is expected by year-end 2026.
The read-through is broadly supportive, not a definitive clinical win. The filing adds early-line evidence to a story that was previously strongest in late-line monotherapy: casdatifan plus zimberelimab produced 86% six-month and 69% 12-month PFS, with a 50% confirmed response rate after 11.5 months of follow-up. 〔0〕 The low 6.7% primary-progression rate also supports the idea that the combination is producing meaningful initial disease control rather than a handful of delayed responses. 〔1〕
| ARC-20 cohort | Follow-up / patients | Key efficacy data | Safety read |
|---|---|---|---|
| Casdatifan + zimberelimab | 11.5 months; n=30 | 6-month PFS 86%; 12-month PFS 69%; cORR 50%; primary progression 6.7% | No unexpected signals |
| Casdatifan + ipilimumab + zimberelimab | 3 months; n=31 treated | 1 of 24 evaluable patients had primary progression, or 4% | No unexpected signals; higher-grade immune events generally consistent with ipilimumab/nivolumab |
| Casdatifan + cabozantinib | 21.3 months; n=43 efficacy population | Median PFS 14.5 months; 18-month OS 77%; cORR 44%; primary progression 5% | No unexpected signals |
| Casdatifan monotherapy | 28 months; n=121 | Median OS 35 months; median PFS 13 months; cORR 36% | Acceptable and manageable across doses |
The second-line combination is the most commercially relevant piece. Casdatifan plus cabozantinib maintained similar efficacy regardless of prior TKI exposure, which addresses a stated weakness of current second-line treatment: patients previously exposed to TKIs often get less benefit from another TKI. 〔2〕 The 14.5-month median PFS and 77% 18-month survival rate are therefore useful support for a TKI-sparing or TKI-enhancing strategy, although this remains a non-randomized cohort rather than a head-to-head demonstration.
The main limitation is maturity and study design, not an obvious safety problem. The triplet data have only three months of median follow-up, and the first-line zimberelimab cohort contains just 30 patients; there is no randomized comparator or registrational endpoint here. The filing does, however, report no unexpected safety signals across the combinations, which removes a near-term obstacle to using these results to support the planned Phase 3 program. The direction was expected from Arcus’s prior development plan, so the surprise is the consistency of activity across treatment settings rather than a wholly new thesis.
Bottom line: This meaningfully strengthens the casdatifan development story across first-, second-, and late-line kidney cancer, but it advances the Phase 3 setup rather than proving the drug’s eventual standard-of-care status.
PEAK-20 Phase 3 initiation, expected by year-end 2026
