Inhibrx's INBRX-106 posts sharp HNSCC efficacy, but toxicity tempers the win

The bar was a stronger confirmatory readout, not proof of concept. The direction was already expected after May's interim data showed a response advantage for the combination; the market was looking for the effect to hold up and translate into durability. The new data clear that bar: the earlier signal has strengthened rather than faded, with the study now supporting a larger HPV+ expansion.

The efficacy result is clearly better than the prior benchmark. INBRX-106 plus pembrolizumab produced a 48.3% confirmed response rate versus 26.5% for pembrolizumab alone, a 21.8-point gap. 〔0〕 (Primary endpoint results)

The HPV+ subgroup is the real value-creating surprise, but it is also statistically fragile. An 80.0% response rate and 90.0% six-month PFS rate are unusually strong against the control arm, and the company is therefore shifting the development emphasis toward HPV+ OPSCC. But those figures come from only 10 combination patients and nine controls, and the reported HPV+ response confidence interval is wide. 〔1〕 (HPV+ subgroup results)

Durability supports the response result rather than merely decorating it. Median PFS was 9.6 months versus 4.9 months overall, while six-month PFS was 72.4% versus 42.8%. In HPV+ disease, median PFS had not yet been reached for the combination versus 4.6 months for control. 〔2〕 (HPV+ subgroup results)

Safety is the principal offset and keeps this from being an unqualified win. In the investor presentation's safety population, grade 3-or-higher adverse events occurred in 58.1% of combination patients versus 47.1% on pembrolizumab, and treatment discontinuations were 35.5% versus 14.7%. The combination also produced more rash, diarrhea, liver-enzyme elevations and infusion reactions. The efficacy signal is strong, but the Phase 3 and regulatory case will need to show that the benefit justifies this added treatment burden. (Safety and tolerability)

The filing changes the program from validation to execution. Inhibrx plans to add approximately 50 HPV+ OPSCC patients, then seek FDA alignment and initiate the confirmatory Phase 3 study. 〔3〕 (Development plans)

Net read: above the clinical bar, with the market's focus now moving to reproducibility and tolerability. This is not a wholly new thesis—the positive direction was already public—but the magnitude, particularly in HPV+ disease and PFS, meaningfully strengthens it. The next value-defining test is whether the expanded cohort reproduces the signal without the current discontinuation and grade 3-or-higher toxicity burden.
